Start/Stop Service
Be prepared to provide the following information:
- Name, Date of Birth, and Social Security Number of all parties listed on the account
- Government issued photo identification card
- Service address
- Mailing address
- Email address (optional)
- Daytime, Evening, and Mobile phone numbers of all parties listed on the account
- Date service is expected to begin or end
- There will be a non-refundable account origination fee of $30 per meter in order to establish an account.
Service Setup
Please complete the "Service On" form below and submit it to our office with required identification.
Discontinue Service
When you wish to have your services turned off, please complete the "Service Off" form below and submit it to our office. Disconnection of service will be no sooner than the next business day.
